How Can I Get Home-Based Care?
To be eligible for home-based care, clients need a diagnosis of physical or cognitive impairment as a result of, or in the presence of, HIV infection resulting in impaired capacity for independent living.
Assessment
If you believe you're eligible, you need to speak with a CSN team member about whether CSN is the right service for you. You can contact CSN yourself or a friend, partner or family can call us on your behalf. If this is the case we'll always ask if this person has your consent and that you know the referral is being made.
If CSN is the right service for you, a CSN Care Liaison Officer will arrange to meet with you, usually at your home, to explain how CSN can help and decide what services are needed.
This may mean that other services are required and, with your consent, we can make the appropriate referral, dependant on your needs and eligibility. CSN will then match you with a volunteer carer who will make contact and arrange a time to meet with you. Together you can negotiate a convenient shift time for you both.
